- Is there lingering tenderness, pressure, or sensitivity?
- Has swelling or drainage occurred?
- Is the patient considering an implant in that area?

These details help place any imaging findings into context. An unusual area on a scan may be less concerning in a patient with no symptoms and normal healing than in someone who has experienced years of unexplained discomfort in the same location.
The Evaluation Includes More Than a Visual Exam
Routine dental examinations naturally focus on the teeth, gums, and visible oral tissues. Holistic dentists also pay close attention to the jawbone beneath those structures.
During the clinical examination, we may evaluate:
- tenderness over the extraction site
- changes in gum contour
- unusual soft tissue texture
- discomfort when pressure is applied
- surrounding tooth stability
- bite changes or altered chewing patterns
- signs of chronic inflammation
No single finding confirms a cavitation. The examination helps determine whether additional diagnostic steps may be useful.
Standard X-Rays May Not Show the Full Picture
Two-dimensional dental X-rays remain useful for detecting many common conditions. However, they compress three-dimensional anatomy into a flat image. Subtle changes in the cancellous bone inside the jaw may be difficult to interpret on routine films.
This does not mean every patient needs advanced imaging. It means that when symptoms, history, or treatment planning raise valid concerns, a more detailed evaluation may be appropriate.
How CBCT Imaging May Help
Cone beam computed tomography, commonly called CBCT, creates a three-dimensional view of the teeth and jawbone. This allows the dentist to examine bone architecture from multiple angles rather than relying on one flat image.
CBCT may help evaluate:
- bone density patterns
- areas of incomplete socket fill
- cortical bone integrity
- changes near previous extraction sites
- hidden infection or inflammatory bone changes
- available bone before implant placement
- nearby nerves, sinuses, and anatomical structures
CBCT findings still require careful interpretation. Variations in bone density do not automatically prove that a clinically significant cavitation is present. The images must be considered alongside the patient’s symptoms, examination, and dental history.

Ultrasound Evaluation Is an Emerging Option
Some biological dental practices use transalveolar ultrasound or related ultrasound technology to evaluate jawbone density and possible areas of reduced healing. These methods are attractive because they do not use ionizing radiation.
Ultrasound may provide additional information about how sound travels through the bone, which can help identify areas that appear less dense than expected. However, this technology is still developing, and its role in cavitation diagnosis is not yet standardized across dentistry.
A responsible holistic dentist should explain the limitations of any emerging diagnostic method and avoid presenting one test as absolute proof of disease.
Holistic Dentists Consider the Quality of Healing
Traditional evaluation may focus primarily on whether the extraction socket has closed and whether there is active pain or infection. Holistic dentistry also considers the biological quality of the healing process.
That includes asking whether the site developed:
- healthy new bone
- stable soft tissue
- adequate blood supply
- low inflammatory activity
- sufficient strength for future restoration
The surface of the gum can close while deeper bone continues to remodel for months. If symptoms persist or future implant treatment is planned, the quality of the underlying bone becomes especially important.
Whole-Body Factors Are Part of the Assessment
Bone healing does not occur separately from the rest of the body. A holistic evaluation may consider whether broader health factors could be affecting recovery.
These may include:
- smoking or nicotine exposure
- diabetes or poor blood sugar control
- nutritional deficiencies
- immune system challenges
- chronic inflammation
- osteoporosis or reduced bone density
- medications that influence bone turnover
- poor circulation
- a history of difficult wound healing
These factors do not prove that a cavitation exists. They help the dentist understand whether a patient may have a higher risk of incomplete or delayed healing.
Symptoms Are Taken Seriously Without Being Overinterpreted
Some patients report discomfort in an old extraction area even when routine imaging appears normal. Others may have unusual imaging findings but no symptoms.
Holistic dentists try to avoid both extremes. We do not dismiss persistent symptoms simply because they are difficult to explain, but we also do not assume that every unexplained symptom is caused by a jawbone cavitation.
A careful evaluation looks for consistency between:
- the patient’s symptoms
- the location of the concern
- clinical findings
- imaging results
- prior dental history
The stronger the connection between these factors, the more meaningful the concern may be.
Cavitation Screening Is Not Routine for Every Patient
Not every person with a past extraction needs cavitation screening. Most extraction sites heal normally and do not require additional investigation.
A closer evaluation may be more appropriate when a patient has:
- persistent discomfort at an old extraction site
- repeated swelling or tenderness
- unusual sensations that have not resolved
- a history of difficult surgical healing
- questionable bone quality before implant placement
- inconclusive findings on standard dental imaging
Screening should be based on clinical need rather than fear.
Evaluation Does Not Automatically Mean Surgery
Finding an area that deserves attention does not automatically mean surgical treatment is necessary. Sometimes the best plan is monitoring, additional imaging, comparison with prior records, or referral for another opinion.
If treatment is being considered, patients should understand:
- what findings were identified
- how certain or uncertain the diagnosis is
- what could happen if the site is monitored
- what the proposed procedure involves
- what benefits and risks are expected
- whether tissue analysis will be performed if surgery occurs
Clear communication is especially important because the terminology and diagnostic standards surrounding cavitations are still evolving.
Why Future Implant Planning Matters
One of the most practical reasons to evaluate an old extraction site is preparation for dental implant treatment. Successful ceramic implant placement depends on healthy, stable bone.
Before recommending an implant, we want to understand:
- whether the bone has healed sufficiently
- whether adequate volume and density are present
- whether inflammation or infection must be addressed
- whether grafting or regenerative support may be needed
- whether the surrounding gum tissue is healthy
A careful evaluation can improve treatment planning and help reduce avoidable complications.
What Makes the Holistic Approach Different
The main difference is not that holistic dentists automatically diagnose more cavitations. The difference is that we are more likely to examine the extraction history, jawbone, healing process, material choices, inflammatory patterns, and whole-body context together.
